Survey data refers to information gathered using survey methods. Examples of survey methods include questionnaires (electronic or paper form), telephone interviews, face-to-face interviews, and focus groups. Surveys are often used as part of patient registries to gather information about patients’ experiences, concerns, unmet medical needs, etc.
